All parents whose children will need to be transported to any location other than their home must fill out a Transportation Request for the upcoming year.
A new form must be completed each school year. They can be obtained at M&M Bus Service, 10606 Hemlock Street NW, Annandale, across from Annandale High School. The offices are open from 7 a.m. to 5 p.m. Monday through Friday.
Because HIPPA (Health Insurance Portability and Accountability Act) restricts access to medical information, it is the responsibility of parents to notify the bus company of any medical conditions their child may have that the company should be aware of. This notification must be provided to the bus company in writing.
Please mail the notification to: M&M Bus Service, 10606 Hemlock Street NW, Annandale, MN 55302.
Most routes will be run as they were last school year. If there is any change for your family, you will be notified by mail or phone.
